I know Eddie Gaedel is the shortest player in history. I have no idea who pitched to him, but I'm guessing they'd have the biggest size difference.

Jose Atluve and Jon Rauch? Rauch is the tallest pitcher ever, and Altuve probably isn't even 5'7", so that would be my guess
