Dang, this is good info. It should help me when I'm (sort of, not in a league) pitching in the hallway at my house, using a pitching door game.

throw what way is comfortable to you, if you've watched... alot of fielders throw a low 3/4 release... just make sure your not having a natural 2seam spin on it and you're fine... (2seam spin and when you're throwing it, it has a chance to move on your teammate and spin out of their glove)

do long toss practices. even though its a "pitchers" practice to get you to be comfortable with your throw.

groundballs, they're going to nab you, its better for them to hit you and you knock it down in front of you than let it go by. You're going to get some bruises... tough it out. You're not going to get a spot on your highschool team if you're scared of the ball. A previous tip earlier, color on a few baseballs so you'll be able to see the spin while it's going through the grass better to better judge what it acts like.

go up to your coaches and ask them whats wrong with your mechanics/ect. I mean we're only going to be able to give you advice about what you know and share. Just ask them what they see wrong with you, and when they tell you, ask them in practice on how to fix it. If you don't show initiative about bettering yourself to them, how will they know you're truly trying to get better?

Sorry if a few things seemed harsh I'm just trying to help.
