The 4 in the center points to the level of the pitch.
Any number that is...
I mean, Lincecum in the game, I had him with a level 3 breaking ball with 4 breaking pitches (including 2 seamer, as you included that in the pic)

As Max has said, a 5 TRJ, which is permentaly popping out to the catcher (sounds like some of the hitters in the MLB). Again, that was MaxDSterling, not me.
